FAIL was the PDP-10 assembler used on the Stanford AI lab WAITS system. It was originally created Phil Petit, and maintained by Fred Wright, Ralph E. Gorin, and many other SAIL hackers. It was ported to TOPS-10, TENEX/TOPS-20, and ITS.
Some source files for early versions of the TENEX monitor were assembled with FAIL, so apparently it was in use at BBN. Later, only MACRO-10 was used.
